Adult Alcohol and Drug Evaluation - $100.00                      Juvenile Alcohol and Drug Evaluation - $100.00

Evaluations last approximately 30 to 45 minutes.  This is a thorough evaluation of ones history and current pattern of use of alcohol and all other chemical substances.  The report is then written and submitted to the appropriate parties, such as probation officers, attorney's or the court.

Adult Mental Health Evaluation - $100.00                           Juvenile Mental Health Evaluation - $100.00

Evaluations last approximately 30 to 45 minutes.  This is a complete bio-psychosocial evaluation and encompasses a mini mental status exam.  The report is then written and submitted to the appropriate parties, such as probation officers, attorney's or the court.

Adult Combination - $150.00                                            Juvenile Combination - $150.00
Alcohol, Drug and Mental Health Evaluation                   Alcohol, Drug and Mental Health Evaluation

Evaluations last approximately 45 minutes to one hour.  This exam is a combination of the two above listed exams.  A thorough evaluation of ones history and current pattern of use of alcohol and all other chemical substances.  Also a  complete bio-psychosocial evaluation and encompasses a mini mental status exam.  All information is contained in one report and submitted to the appropriate parties, such as probation officers, attorney’s or the court.

Multiple Offender Program - Alcohol and Drug Evaluation - $150.00

This is a state evaluation that is overseen by the Department of Human Resources, Multiple Offender Program and can assist a DUI offender that has multiple DUI’s in getting his/her driver’s license reinstated.  This evaluation is regulated by the Department of Human Resources and the report is written in DHR format.  This is an intensive evaluation that is submitted to the Department of Human Resources as well as the Defendant’s Attorney and Probation Officer, if applicable.       

Domestic Violence Evaluation - $100.00

Evaluations last approximately 45 minutes to one hour.  The Domestic Violence Evaluation consists of 76 questions and takes about 15 to 20 minutes to complete the written portion followed by a short discussion with the clinical evaluator.  The evaluation measures Truthfulness Scale, Violence Scale, Alcohol and Drug Scale, Control Scale and Stress Coping Abilities.  All information is contained in one report and submitted to the appropriate parties, such as probation officers, attorney's or the court.
